**Job Title: Care Coordinator (Hybrid)**

**Job Summary:**
Our organization, with the support of regional advisory councils and government funding, is committed to implementing a comprehensive care coordination model for Commercially Sexually Exploited Youth (CSEY). Our approach is collaborative, consensus-driven, and focused on providing tailored, trauma-informed, and holistic resources through a coordinated network of providers.
The primary role of the CSEY Care Coordinator is to implement our care coordination program, facilitate regional consensus-building, and ensure compliance with our policies and procedures. This role involves crisis response, case management, stakeholder engagement, data evaluation, and event planning.

**Working Hours and Conditions:**
- Generally, Monday through Friday, 8:30 AM - 5:30 PM
- Shared 24/7 on-call responsibility with another regional care coordinator
- Anticipated 45-50 hour work week, with some travel possible
- Primarily remote work, with some office-based tasks and remote night-time responses required
**Job Responsibilities:**
- Build consensus among regional partner agencies to establish protocols
- Implement our care coordination program in accordance with government expectations
- Share 24/7 remote care coordination line with Lead Care Coordinator
- Complete CSE Identification Tools (CSE-ITs) as needed
- Secure and retain release of information and consent for care coordination services
- Encourage engagement of CSEY advocacy services
- Facilitate rapid response meetings, service staffing meetings, and family engagement meetings
- Schedule and facilitate regular meetings of advisory councils in service region
- Maintain confidentiality of sensitive information
- Support regional care coordination team in promoting and hosting awareness events and education/training events
- Facilitate case analyses by regional care coordination team
- Maintain open communication with the Care Coordination Program Director
- Attend weekly meetings with co-CSEY Care Coordinator and Program Director
- Document all incoming referrals, intakes, meetings, service plans, outgoing referrals, and communications in a timely manner in our electronic case management system
- Develop and maintain good working relationships with essential regional partners
- Complete all required training in a timely manner
- Respond appropriately to allegations of abuse, following mandatory reporting requirements

**Qualifications:**
- Bachelor's degree in social work or related field
- Experience working with youth who have experienced commercial sexual exploitation
- Experience working collaboratively with regional partner agencies
- Proficient in facilitating awareness presentations and trainings
- Excellent verbal and written communication skills
- Demonstrated history of achieving positive outcomes through effective group facilitation and stakeholder engagement
- Experience with documentation in a cloud-based case management software
- Experience facilitating protocol development
- Trained and experienced in trauma-informed care
