What You Will Do as a Chronic Care Coordinator (CCC):
As a Chronic Care Coordinator, you will play a vital role in maintaining strong relationships with both our staff and the Chronic Care Management (CCM) team. You will serve as a liaison for patients, navigating between various specialty departments, and continuously addressing the needs of patients, caregivers, and families in collaboration with a multidisciplinary team.
A Day in the Life of a Chronic Care Coordinator:
Manage a caseload of patients assigned by the Chronic Care Program Manager.
Provide monthly non-face-to-face care coordination services for Medicare beneficiaries with multiple chronic conditions.
Develop patient-centered care plans that consider physical, mental, cognitive, psychosocial, functional, and environmental needs.
Facilitate care transitions between healthcare providers and settings, including referrals to external providers.
Follow up with patients after emergency department visits and discharges from hospitals, skilled nursing facilities, and other healthcare settings.
Coordinate care with home and community-based clinical service providers.
Identify resources available to patients, caregivers, and families.
Communicate patient needs and concerns to the multidisciplinary team and coordinate care accordingly.
Document monthly contact and care management notes in the electronic health record.
Submit monthly billing reports to the Chronic Care Program Coordinator.
Attend scheduled monthly team meetings.
Perform other duties as assigned.
Required Qualifications:
Current licensure as a Registered Nurse or Licensed Practical Nurse in the State of North Carolina.
Experience:
A minimum of 2 years of Case Management experience is preferred.
Shift: Day Shift (Monday through Friday)
Pay Type: Hourly (Non-Exempt)
