Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly xx210 and below Per hour not OT

Holiday Must be 1.5x the regular rates Per holiday hour no OT

Schedule/Availability Requirements:

ASAP as soon as credentialed 6 months to a year

Shift times: Weekdays: 8:00 am to 5:00 pm with a 1 hour lunch

Frequency: Monday Friday every week no call would also consider someone who could do only a few days/week

Facility Location: 1595 Soquel Suite 400 Santa Cruz CA

Setting: Internal Medicine will consider a strong FP but that is not ideal.

FTE: 1.0

# Providers Needed: 1

Duties: Typical IM clinic population average age 70. About half male/female with multiple comorbidities such as diabetes arthritis heart disease chf etc

Required Procedures: N/A

EMR: Cerner

Patients per Day: Other providers seeing 1218 a day depending upon complexity (includes new patients)

Support Staff: MAs MORs APP (one in clinic) and RN assigned to clinic

Reason for Coverage: Provider Retiring

Deal Breakers Not board certified

Travel Preferences:

Preferred Lodging: Our Locums stay in hotels or in AB&B up to the CommonSpirit cap

Air Travel: Would prefer someone local but wouldnt rule out someone who wants to fly home on the weekends.
